What is the dual angle layout for a ball drilled 4x4x2 with the Storm system?

Approximately 55 X 4 X 30 with a 3" pin as far as I can tell.

I get 42 x 4 x 34.
The Pin to CG distance (3" in Rob's example) is not a factor.

Thanks, Mike. So, the second "4" in the Storm formula is the distance from the PAP to the mass bias, or 6 3/4" from the pin spot?

Yep it is Rob.
The 4X4X2 layout is shown right in the Storm PSA Lighting Strike and PSA drill sheets.

They use the same steps for a Symmetrical also, only the first step is :
(STEP #1 Find the point 6-3/4” from the Pin through the CG and a mark this with an “X”. This indicates the PSA of the undrilled ball.)
See Storm Symmetrical drill sheet here:

PSA Drilling Instructions
4" x 4" x 2" Example:
4" Pin to PAP x 4" PSA to PAP x 2" Pin Buffer to VAL
Steps:
1. Draw an arc 4" from the center of the Pin.
2. Draw a second arc 4" from the center of the Preferred Spin Axis (PSA) that intersects the first arc from step 1.
Where these arcs intersect is your bowler’s Positive Axis Point (PAP).
3. Now that you have found the PAP location, you need to find the Pin Buffer. In this example, use a 2" Pin Buffer. Draw a second arc 2" from the Pin.
4. Draw a line through the point where the arcs from steps 1 and 2 intersect (PAP). This line should be tangent* to the Pin Buffer. This line is your bowler’s Vertical Axis Line (VAL).

That spreadsheet looks familier.
I think Kelly T contributed the conversion from Storm to Dual, and I contributed the Dual to Storm. And Erik combined them into the one spreadsheet you linked.
