does anyone have a link to a good player waiver form that I can use for a league I'm starting? Thanks!

A waiver for what? I've never had to sign a waiver for any league I've ever bowled.

Ok, fair enough. I have drafted this set of rules for the league. Thoughts?


D&D BOWLING RULES & REGULATIONS
1. Each individual participant must print & sign his/her name on the team roster form to be eligible for participation in Drink & Drag leagues & events. This form is available at all league games. Team captains are responsible for having all team members sign this roster form. Please be aware that there are inherent risks to participating in any athletic activity. D&D does not provide insurance for the participants of our leagues. Each player as a result of participation in the leagues assumes all liability and risk of injury. It is strongly recommended that each player obtain medical and liability insurance prior to participating in any athletic or league activity.
2. All team rosters are deemed final at the end of regular season play. Players must participate in three or more matches during the regular season to be CONFIRMED on the team roster. Only CONFIRMED team roster players are eligible to play during the post-season. As a rule, each participant is allowed to be on one team roster per league/level/night. We are happy to allow people to play on more than one team/night if facility space allows and if you inform D&D prior to the start of the league season. Any player not confirmed on a team roster is not eligible to play on that team during the post-season. Any player who is confirmed on multiple rosters during the regular season MUST choose only one team to play on following the initial round of post-season play.
3. Substitutes that have played with your team during the season are not allowed to participate in the playoffs in any of our D&D leagues (it's a basic fairness issue for all of the other teams in your league). The Honor Code is always in effect here as well.
4. Teams: All D&D bowling teams consist of three or four bowlers on the lane at any one time.
5. League Matches: All games will be coordinated/officiated by bowling alley staff in conjunction with Drink & Drag rules & regulations. If you have any questions during league play, please contact D&D Fun Director Justice Counce at 702-327-6844 or justice@drinkanddrag.com
6. Start of play: Your match begins at or before your scheduled start time each league night. All bowlers must be on your lane & prepared to begin bowling at or before the league start time.
7. Length of play: Each league match consists of: THREE games per team per night.
8. Scoring: League scoring will be done on the following basis: Two (2) points are awarded to the winner of each team game and One (1) point is awarded to the team with highest total wood for all 3 games (total pinfall total for all 3 games).
9. League Champions: The league champion will be determined by the match results throughout the league season. After week 6, a position round will implemented. The team with the most points at the end of the league season will be awarded the league championship.
10. Prizes: Prizes will be awarded to the 3 teams with highest point totals. Prize payouts will go as follows: 60% to 1st place, 18% to 2nd place, and 8% to 3rd place. Separate prizes will be awarded to the Highest Scoring Bowler and the Most Improved Bowler.
11. Injuries: Please report all injuries to the on-site Supervisor. Basic first-aid supplies are available.
12. Behavior: The bowling alley staff has the right to warn or eject any player for inappropriate or excessive behavior at any time. Keep it clean & fair out there!! This is all about having a good time bowling with old & new friends, have fun!

So your wanting a liability waiver everybody can sign. Just do a google search on "liability waiver form" there's bunches of them.

1 & 2 sound confusing regular season and a "post season bowling"??. liability waivers

This is worded badly. It makes it sound like if you subbed on a monday league. But were a regular team member on a friday league, you can't bowl on fridays playoffs

if your going to allow a 3 person team , you'll need a vacancy score for the empty 4th position.

If a team has a bowler absent and no sub found, how is that scored?

What if a bowler shows up late, is he allowed to make up frames or are they scored a zero?

I am assuming there is no handicap since there is none identified in the rules, if there is a handicap may want to make mention of this.
